I understand the following: • This discussion will not impact my current or future health coverage unless I decide to enroll in a plan. • I am not obligated to enroll in any plan, and automatic enrollment will not occur without my explicit consent. • This appointment may take place 48 hours from now or sooner if an exception applies under CMS guidelines. I further acknowledge that I can contact Medicare directly for additional plan options or information by visiting Medicare.gov or calling 1-800-MEDICARE (TTY 1-877-486-2048).
